3M™ Fall Protection has identified a potential manufacturing issue with a limited number of 3M™ Protecta® Rebel Self-Retracting Lifelines (with galvanized or stainless-steel lifelines) produced between October 14, 2019 and February 25, 2020.

3M Fall Protection has detected a small quantity of DBI-SALA® 16 Ft. Talon™ Self Retracting Lifelines that are defective due to an assembly error. This notice only applies to the 16 ft version of the Talon; no other versions of the Talon are affected by this condition. There have been no reports of fall-related injuries associated with this condition. However, this assembly error would result in the unit not arresting a fall, which could result in serious injury or death.

3M Fall Protection has determined that the barrel mounts do not fully meet all load claims in all adjustment positions as represented in the “Instruction for Use” (IFU) manual and on product labels. There have been no reported accidents or injuries associated with this issue. This issue only affects the following part/model numbers identified on the product labels of the units: 8512285, 8562285, 8510715, and 8516088.
